CLINICAL CASE MANAGER

Saint Vincent's Home has one full time position available for a Clinical Case Manager.

Responsibilities include: Providing case management and brief supportive counseling; completing CANS assessments and other diagnostic assessments; co-facilitate group sessions; chair treatment plan meetings and case conferences; flexible hours required including participation in after hours on-call rotation.

Requirements:
Clinical master's degree required; licensed or license eligible and two years of experience serving children and families, preferably with some residential experience.
